Building a Patient 360 Platform for a Multi-Hospital Network
Healthcare
What They Were Facing
CareVista operated 12 hospitals and 80+ outpatient clinics with patient data siloed across three separate EHR systems, a legacy CRM, and dozens of departmental spreadsheets. Care teams lacked a unified view of patient interactions, leading to duplicated tests, missed follow-ups, and a 14% readmission rate that was well above the national benchmark.

How We Solved It
AQBEE implemented Salesforce Health Cloud as the central patient engagement platform, integrating all three EHR systems via MuleSoft FHIR APIs for real-time clinical data exchange. We deployed Data Cloud to resolve patient identities across systems, creating a single unified profile for each of the network's 2.3 million patients. Einstein AI risk stratification models were built to predict readmission probability, triggering automated care plan assignments and outreach workflows through Marketing Cloud. An Experience Cloud patient portal provided self-service appointment scheduling, lab results, and secure messaging.
The Impact
30-day readmission rate dropped from 14% to 9.7% within the first year through AI-driven risk identification and proactive care coordination.
Unified 2.3 million patient records from disparate systems into a single golden record using Data Cloud identity resolution.
Automated outreach campaigns closed 42% more preventive care gaps compared to the previous manual process.
Patient NPS improved from 34 to 53 driven by the new self-service portal and proactive communication.
“AQBEE transformed how our care teams see and serve patients. For the first time, a nurse in our Portland clinic can see what happened at our Seattle ER last week — in real time. The Einstein AI models are flagging at-risk patients we would have missed entirely, and our readmission numbers prove it.”
